• My child’s video or voice (if submitted or recorded)

• My child’s story or testimony (if submitted)

• My own name, photo, or testimonial as the parent/guardian

I understand that these may be used in promotional materials such as:

• Whatnot streams and livestreams

• Social media posts (e.g., Facebook, Instagram, YouTube)

• Printed newsletters, flyers, or donor letters

• The Kardz For Kids INC website or digital ads

I acknowledge and agree that:

• Only my child’s first name or nickname will be used (never full names or addresses)

• Kardz For Kids INC will never sell or share this media with outside parties

• I will not receive payment or royalties for use of submitted media

• This consent is ongoing, but I may revoke it in writing at any time by emailing hello@kardzforkidz.org

• Media already shared or published prior to revocation may remain in use
